The Heart-Cancer Connection Researchers Did Not Expect to Find

A new UCLA Health study published in the Journal of the American Heart Association found that small, early changes in heart structure — visible on a cardiac MRI — are linked to a higher risk of developing certain cancers later. The changes involve a process called cardiac remodeling, which means the heart is quietly shifting in size or function. These shifts happen before any obvious disease appears, which is what makes this finding so striking.[1]

The strongest link in the study connected higher left ventricular mass — that is, more muscle mass in the heart’s main pumping chamber — to a greater risk of breast cancer. Separately, reduced left atrial strain, which measures how well the upper left chamber of the heart squeezes and relaxes, was strongly tied to colorectal cancer risk. Patients with better left atrial function had substantially lower colorectal cancer rates.[1]

What the MRI Numbers Actually Showed

The UCLA team analyzed cardiac MRI data from more than 4,500 patients. During the follow-up period, 790 new cancer cases were recorded, including breast, colorectal, lung, and prostate cancers. As the measures of cardiac remodeling worsened, cancer rates climbed. Researchers adjusted the results for known cardiovascular and cancer risk factors — things like age, smoking, and obesity — and the associations held up.[1]

Lead researcher Dr. Xinjiang Cai, a UCLA Health cardiologist, put it plainly: “Structural and functional changes in the heart may occur alongside — or even before — biological processes linked to cancer development.” That is a remarkable statement. It suggests the heart may function as an early warning system, flagging systemic trouble before a tumor ever forms.[1]

Blood Tests Tell a Similar Story

This MRI study is not UCLA’s first signal pointing toward a heart-cancer link. A separate UCLA study found that tiny elevations in two cardiac blood markers — high-sensitivity cardiac troponin T, which detects heart muscle injury, and N-terminal pro-B-type natriuretic peptide, which detects heart stress — were strong predictors of future cancer risk, even in people with no history of heart disease. Elevated levels of both markers were tied to colorectal cancer risk, while the natriuretic peptide marker alone was also linked to lung cancer risk.[3]

Participants in that blood marker study were followed for a median of nearly 18 years. The fact that two very different measurement methods — MRI imaging and blood tests — are both pointing toward the same heart-cancer connection gives this line of research real weight. It is not a fluke of one method or one dataset.[3]

Why This Is Not a Green Light for Panic or Policy Changes Yet

The researchers themselves are careful here, and that honesty matters. “These findings represent associations and do not establish causation,” Dr. Cai said. The results do not mean a thicker heart muscle directly causes breast cancer, or that poor atrial function directly triggers colorectal tumors. Observational studies like this one can show patterns, but they cannot prove one thing caused another.[1] Independent validation in different patient populations still needs to happen before these markers could responsibly guide clinical cancer screening decisions.

That said, the practical opportunity here is real and worth taking seriously. Cardiac MRI scans are already ordered for heart disease risk. If those same scans can flag patients who need closer cancer surveillance too, the cost of that added insight is essentially zero. You do not need to wait for perfect certainty before asking your doctor whether your heart scan results tell a broader story about your health.

Right now, cardiac MRI is used to catch heart failure risk early. Dr. Cai’s team argues those same imaging markers may also help identify people at elevated cancer risk — giving doctors a two-for-one window into a patient’s future health.[1] That dual-use potential is the real headline. Heart disease and cancer are the two leading causes of death in the United States. The idea that one scan might inform risk for both changes how we should think about preventive imaging entirely.
